We welcome new patients who live within the practice boundary. An indication of this boundary is available on this site under the heading Practice Area.
In order to join the SA1 Medical Centre you will need to complete our registration forms.
Also, before joining the practice you will be asked to fill in a medical questionnaire and lifestyle questionnaire, and attend a “new patient” appointment.
It is important that you keep this appointment, as it enables us to record information regarding your medical history, allergies and family history.
This gives you the opportunity to provide us with all the information we need to provide you with the best possible care. New patients will be given a Questionnaire on registering with the surgery. If you prefer, please click on the link below and print off the appropriate form which you could complete beforehand.
